You can get Form code to get subscriber information by going to Autoresponder name > PRIVATE SETTINGS > HTML code.
Copy and paste on your web site. The more Form fields you add, they appear in this code.
You can generate the form code to keep on your web site. You can customize like changing background color, font etc. Be careful not to remove input tags. Form won't work.
After changing the layout, copy and paste it on your web site.
Sometimes Frontpage introduces extra < form tags in to your form. So you be watchful while using FP to upload the form code to your website.
Use SSI to keep the form on everypage of website.

The URL has ' / ' at the end?

If not, keep a ' / ' and save the Systen settings. Go to 'HTML code' and copy and paste the code again on your web site.
